How to choose the right adult diaper size

Golden Years · 6 September 2026

Most leaks and most discomfort come down to one thing that is easy to fix: the size. Here is how to get it right the first time.

Most leaks, and most of the discomfort people blame on the product, come down to one simple thing. The size is wrong. A diaper that is too big gaps at the legs and lets fluid escape. One that is too small feels tight, marks the skin and fills too fast. Getting the size right fixes more problems than any other single choice.

Measure, do not guess

Adult diaper sizes follow the waist and hip measurement, not clothing size or body weight alone. Use a soft tape measure around the widest part of the hips and around the natural waist, and use the larger of the two numbers to pick the size.

  • Medium usually fits roughly 71 to 90 cm (28 to 35 in)
  • Large usually fits roughly 91 to 110 cm (36 to 43 in)
  • Extra large usually fits roughly 111 to 130 cm (44 to 51 in)

These ranges are a starting point. Always check the range printed on the pack, since it varies a little between products.

Check the fit, not just the label

A good fit sits snug at the waist with no gap, and the leg cuffs sit inside the groove of the thigh, turned out so they can catch fluid. If you can slide a flat hand in easily at the waist, size down. If it digs in or leaves deep marks, size up.

When you are between sizes

If someone falls between two sizes, the right choice depends on the body shape. A fuller waist usually does better in the larger size, while a slimmer, taller frame often holds better in the smaller one. Order samples in both and try them for a full day and night before committing to a bulk order.

Sizes change, so re-check

Weight changes, illness and recovery all shift the right size over time. Re-measure every few months, and any time the fit starts to feel off. The correct size is the one that fits today, not the one you bought last year.
